Trauma-Informed Care At Every Stage
Lack of access to quality behavioral healthcare is one of the root causes of homelessness. Project HOME, under its Federally Qualified Health Center (FQHC) status, provides access to trauma-informed behavioral healthcare for those currently experiencing homelessness, those recovering from homelessness in our residences, and low-income residents of Lower North Philadelphia.
Care starts on the street level, with training for our Outreach teams who work to build trusting relationships with those living unsheltered. At the Hub of Hope engagement center in Suburban Station, all patients who come for a medical visit undergo a basic behavioral health screening in order to best serve their needs. A psychiatric nurse practitioner is on staff to provide further care. Our Stephen Klein Wellness Center (SKWC) has a team of behavioral health professionals who specialize in serving those who have a more difficult time accessing care due to homelessness, mistrust of medical professionals, behavioral health issues, and substance use disorder. In our residences, 53 percent of residents report a mental health condition and 11 percent report a substance use disorder.
Our case management staff work with residents on their recovery goals and connect them to services at SKWC and other partner providers in the community. Medication Assisted Treatment (MAT) for opioid use disorder is offered both at the Hub of Hope and at SKWC. Project HOME is a designated Center of Excellence for treatment of opioid use disorder.
Primary Care with a Behavioral Health Specialist
Project HOME’s Integrated Medical and Behavioral Health Services department adds a behavioral specialist to a patient’s primary care team. Our primary care is focused on the physical health of the whole person—which includes disease prevention, the diagnosis and treatment of illnesses, and health maintenance and counseling—and is provided by board-certified physicians, nurses, and medical assistants.
Our clinical social workers supplement that work by developing a problem- focused treatment plan aimed at improvement in patients’ coping/life skills, treatment adherence, and other goals related to better quality of life and health outcomes.

Project HOME's Healthcare Services is a Federally Qualified Health Center (FQHC) and we are able to provide our services regardless of a person’s ability to pay for the services. In cases where an individual lacks health insurance, we offer a discounted sliding fee program.
Project HOME receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims including medical malpractice claims for itself and its covered individuals.
