We Remember - Collecting Names for 2016 Homeless Memorial Day

The Philadelphia Homeless Memorial Day Planning Committee is reaching out to you to help gather the names of homeless and formerly homeless women, men, and children who died in 2015 (and since last year’s remembrance on 12/18/14). This year’s Homeless Memorial Day will take place on Wednesday, December 21. The location is Thomas Paine Plaza and the time is 5:00 p.m. Please join us.
 
We want to have as thorough a list of names as possible to remember and honor this year. We need the names of people who were either experiencing homeless at the time of their death or who previously experienced homelessness. Please submit those names to [email protected] or call Heather Bargeron at 215-232-7272, ext. 3061 by Friday, December 2. Please include as much information as you have and feel comfortable sharing about each individual (see list below or download the form at bottom of this page). We are requesting this information only to verify identity; only the names will be publicized at the event.

We need your help to make sure that we are including ALL the people in our community who were homeless and formerly homeless who passed away this year. Please share this email with your colleagues.

•    Were they homeless at time of death? (meaning were they living on the street, in emergency housing, in  a safe haven, or couch surfing?)  
•    Organizational Affiliation
•    Date of Birth      
•    Date of Death
•    Place of Death   
•    Manner of Death             
•    Cause of Death
•    Contributing Factors
•    Net of Kin Notified?
•    Race/Ethnicity
•    Veteran?      
•    Last 4 digits of SSN  (so we can find out if they were a vet)
•    Anything else we should know?
•    Submitter's Name/Email/Phone/Agency
