Part-Time Registered Nurse
The Part-Time Registered Nurse must support and demonstrate the mission and values of Project Home; Demonstrating compassion and sensitivity to Project H.O.M.E. and neighborhood residents, maintaining confidentiality regarding neighborhood residents, promote awareness towards a healthy work environment and good communication among neighborhood residents and will take a pro-active stance in dealing with the disenfranchised by addressing socio-economic needs in addition to clinical needs The Part time RN w ill work in collaboration with a team of health care providers to provide quality care to Project H.O.M.E. and neighborhood residents, especially those who are medically fragile. The RN will initiate pre-hospital discharge planning for selected patients of the St. Elizabeth's Health Center / Jefferson Family Medical Associates; assess and monitor patients who are discharged from the hospital; conduct home visits as needed. Other responsibilities will also include assisting in the coordination of care of patients as assigned; Provide patient care during walk-in clinic sessions (Tuesdays, 9-11AM and/or Wednesdays, 2-5PM); Supervise student nurses; Provide health education at Project H.O.M.E. sites as needed.
Qualified candidates must have:
- RN Diploma or Associate Degree;
- Bachelor's Degree preferred;
- Must provide proof of current RN licensure in Pennsylvania ;
A minimum of 1 year clinical experience;
The ability to establish and strengthen relationships within the community and with medical partners;
Excellent written and verbal communication skills;
Strong computer, organizational and administrative skills.
The ability to successfully manage multiple tasks simultaneously and to work independently or with a team.
Schedule : 10 hours per week for one year
