Case Manager - St. Columba's

St. Columba's is a safe haven residence for 40 chronically homeless men with severe mental illness, and some co-occurring substance abuse disorders. The Case manager's primary responsibilities will be to maintain a caseload of ten to twelve residents: assist with personal finances, secure benefits, assist men with daily living skills, act as a liaison with outside service providers, maintain case records, file systems, and maintain open communication with outside agencies.  On call duties are required.  Services are provided on a 24-hour basis, including case management, supportive services and on-site medical care.   Qualified candidate must have a BA in Social Work or a related field, prior caseworker exp., excellent verbal and written communication skills, a willingness to listen and respond to others with compassion, knowledge and experience with mental illness, issues regarding homelessness, addictions & men, and proficient computer skills (Microsoft Office Applications) required.  A valid driver's license required.  Schedule: M-F 10 am - 6 pm, rotational evenings on-call.   Location: 4133 Chestnut Street
